A Complete List of CHAN’s Full and Affiliate Members

Many of the FQHC sites are located in officially designated medically underserved areas and/or health professional shortage areas. Currently, the HCCN coverage encompasses four counties in southern New Hampshire (representing more than half of the state’s population) and six centers in Texas.

Membership is available at two levels: Full Membership and Affiliate Membership.

Full Membership is open to nonprofit primary health care agencies and affords access to all HCCN services and resources. Full Members have representation on the Board of Directors.

Affiliate Members do not participate in the Board of Directors. Some Affiliate Members house their own IT infrastructure and others access CHAN systems.
