About Us

For more then 12 years, CHAN has methodically built an infrastructure that now affords us innovative opportunities.  Our cumulative endeavors, particularly in the Health Information Technology (HIT) arena, enable us to consider and provide enriched patient experiences and to support quality care through HIT.

Milestones:

  • Implemented Centricity Physician Office EMR (formally Logician) electronic medical record system that is standardized and centrally managed. 100% of the member providers use the system for patient care.
  • Selected and implemented Centricity Physician Office Practice Management (formally Millbrook)  It is fully integrated with the EMR and enables HIPAA compliant business transactions.
  • Implemented secure web-enabled technology tools for authorized users (patients and providers) to interact with the health center for services and information.
  • Standardized financial definitions and practices including chart of accounts, status and performance report and compliance oversight procedures.
  • Adopted and implemented clinical standards for preventive health care services and the management of common chronic health conditions (Diabetes, Asthma, Depression, Cardiovascular). 
  • Integrated sub-specialty services and programs for oral health, behavioral health and obstetrics into the primary care settings for the underserved populations.
  • Organized and provide structured technical assistance and operations consulting services to other Health Centers and Networks that may be interested.
