The network
partners provide primary health care services to low-income, uninsured
and Medicaid eligible residents. Many of the sites are located in
officially designated Medially Under-served Areas and/or Health
Professional Shortage Areas. Currently, the Network coverage
encompasses four counties in southern New Hampshire representing more
than half of the state’s population.
Membership is
available at two levels: Full Membership and Affiliate Membership. Full Membership is open
to non-profit primary health care agencies and affords access to all
Network services and resources. Full Members have representation on the
Board of Directors. Affiliate members do not participate on the Board
of Directors. Some Affiliate members house their own
infrastructure and others access CHAN Network benefits. Each are
equally
important to the depth and richness of the Network.
