click above to go to MCHC's website Manchester Community Health Center1415 Elm Street, Manchester, NH 03101 Phone: (603)626-9500 Fax: (603)626-0899 Manchester Community Health Center (MCHC) was established in 1993 particularly to provide family oriented primary care services to those in greater Manchester who are uninsured, under-insured or lacking access to sources of quality health care. It is the premier facility rendering primary care to area residents regardless of age, income or ethnicity. MCHC is a federally-qualified health center and holds the highest rating by the Joint Commission on Accreditation of Healthcare Organizations (JCAHO). MCHC was the first community health center in the state to successfully pass the JCAHO review process. Staff members include: Family Practice Physicians, Internists, Family Nurse Practitioner, Certified Nurse Midwife, Physician Assistants, Family Community Health Coordinator, Registered Nurses, Outreach Worker, Nutritionist and various other clinical, executive and administrative staff. (Total staff of approximately 50 individuals). Services are provided on a discounted fee scale, based upon the patient's income and family size, and address the patient's medical and social needs. Basic Services offered include: Family Medicine Perinatal Care Health Education Social Services Coordination Preventive Screening Counseling Services Nutrition Translation Outreach Services Bi-lingual Case Management MCHC currently serves about 9,000 active patients of whom nearly 30% do not use English as their primary language. The Health Center is sensitive to issues regarding culture and poverty, and employs several bilingual/bicultural staff (approximately 25%) members on site. It is open Monday - Friday and one evening (Thursday) per week. The Health Center is located on a city bus route and is handicapped accessible. |
